Derma Roller Before and After Hair Growth

Honest derma roller before and after hair photos show a modest but real change: less scalp showing through at the crown, a hairline that looks fuller rather than restored, and short new hairs that were not there before. They are taken six months apart in the same light, and they come from people who rolled weekly and used minoxidil daily. This post explains what a genuine before-and-after looks like, how to take your own so they mean something, and how to spot the faked ones that fill social media.

What a real before and after looks like

Start with what the evidence supports. The 2013 trial by Dhurat and colleagues found that men who added weekly dermarolling to 5% minoxidil had about four times the hair-count gain of men on minoxidil alone over 12 weeks. That is a large difference in a hair count done under magnification. In a phone photo, a change of that size looks like this: the crown goes from “you can clearly see scalp” to “you can see some scalp under bright light”. The hairline goes from a thin, see-through fringe to something denser, with a few short dark hairs a little forward of where the hair used to stop.

It does not look like a bald man becoming a man with a full head of hair. Rolling works on follicles that are still alive and producing thin hair. It does not bring back follicles that have been gone for years. A before-and-after that shows shiny bald skin turning into thick hair is either a transplant, a hair system, or a lie.

The stages you will see in your own photos

If you photograph yourself at the start, at three months and at six months, this is roughly what the three images will show for a man with thinning at the crown who rolls weekly and uses minoxidil daily.

Photo What it shows How you will feel about it
Start Scalp visible at the crown, fine hairs, maybe a thinning hairline. Motivated.
Month 3 Slightly less scalp visible. Some short new hairs. Hard to be sure it is real. Unsure, possibly disappointed.
Month 6 Noticeably less scalp showing. New hairs have grown out and darkened. A friend would notice. Convinced, or, if nothing changed, ready to move on.

The reason month three looks underwhelming is simple: a hair that starts growing in month two is barely a centimetre long by month three. By month six it has had time to grow out, thicken and pick up colour. The full timing is in our post on how long does derma roller take for hair growth.

Somewhere in the first month, before the three-month photo, many people also go through a short shedding phase where more hair comes out in the shower. It settles on its own and it does not show in a photo taken at three months. Do not let it push you into quitting before the first comparison.

How to take before and after photos that mean something

Almost every “it didn’t work” and half the “it worked amazingly” messages we get are from people comparing photos taken under completely different conditions. Hair density is very sensitive to light angle, hair length, whether the hair is wet, and how it is combed. Fix all of those and the comparison becomes honest.

  1. Choose one spot in your home with a fixed light source, such as a bathroom with the ceiling light on and no window light. Use it every time, at the same time of day.
  2. Have the same person take the photo from the same distance. Mark where they stand if you have to.
  3. Hair dry, at roughly the same length, combed the same way. If you get a haircut, wait a week before the next photo.
  4. No flash. Flash flattens hair and hides thinning.
  5. Take three angles: crown from directly above, hairline from the front with the head tilted slightly down, and each temple from the side.
  6. Photograph at the start, at month three and at month six. Not every week. Weekly photos show nothing and make you doubt the process.

Then look at scalp visibility, not hair volume. Ask: can I see more or less skin through the hair in the same spot? That is the question that matters.

How to spot fake before and after photos

Search for derma roller results on Instagram or TikTok and you will find hundreds of dramatic transformations, most of them selling a roller and an oil as a bundle. Here is how they are made.

  • Different lighting. The “before” is shot under a bright overhead light, which makes any scalp glow through. The “after” is shot in soft front light. Same hair, different photo.
  • Wet versus dry. Wet hair clumps and shows scalp. The before is wet, the after is dry and blow-dried.
  • Different hair length. The after has two months of extra length styled to cover the thin area.
  • Hair fibres or scalp concealer. Coloured powders and fibres sold in Pakistan for a few hundred rupees make a thin crown look full in a photo instantly.
  • Different people. Common in marketplace listings. Look at ears, moles and hairline shape.
  • Transplant results relabelled. Dense new growth in a sharp line across a previously bald forehead is a transplant, not a roller.

A genuine result is undramatic in a single photo and convincing across six months of consistent ones. If a seller’s photos look too good, they are.

Results by area of the scalp

The crown responds best in our experience, partly because it usually has the most surviving thin hairs and partly because it is easy to roll evenly. Customers most often report a visible improvement there first.

The hairline is harder. Once the hairline has moved back and the skin in front of it is smooth, the follicles there are gone. What rolling can do is thicken the thinning zone just behind the current hairline, which makes the whole front look denser. Our guide to using a derma roller for receding hairline thinning explains how to roll the temples without irritating the forehead.

Diffuse thinning across the whole top, common in women and in some men, tends to show as a general improvement in thickness rather than growth in one spot. The before-and-after is subtle and photos from above show it best. Our post on the derma roller for hair density covers that pattern.

What produced the results that customers send us

The photos we receive that show a real change all come from the same routine. Weekly rolling with a 0.5mm roller. Minoxidil 5% applied after rolling and every other day. Six months minimum before judging. Roller replaced every two to three months because needles dull after roughly 10 to 15 uses.

The roller used in the clinic trial was 1.5mm. We recommend 0.5mm for home use because it can be used weekly without bleeding or scabbing and because it gets minoxidil into the scalp properly, which is a large part of why the combination works. Our 0.5mm derma roller for PKR 799 has 540 titanium alloy needles, a non-slip handle and a storage case, and it ships free with cash on delivery across Pakistan.

The photos that show no change come from people who rolled without minoxidil, rolled for six weeks and stopped, pressed hard enough to bleed, or had bald skin rather than thinning hair to begin with. The overall picture of what to expect is in our post on derma roller hair loss results.

Frequently asked questions

How many months of derma rolling before I see a difference?

Roughly three months for the first change, six months for a clear one. Compare photos at those two points and not in between.

Why does my before photo look better than my after?

Almost always lighting, hair length or wet hair. Retake both under the fixed conditions described above. If the after still looks worse and you are shedding heavily past two months, stop and see a dermatologist.

Do derma roller results last?

Only as long as you keep the routine going, at a reduced frequency, and keep using minoxidil. Stop everything and the follicles drift back within six to twelve months.

Can a derma roller regrow a completely bald crown?

No. It thickens thin hair and reactivates resting follicles. Shiny skin with no fuzz means the follicles are gone, and a transplant is the only option that grows hair there.

The evidence, the needle-length choice and the full routine are all in our guide to the derma roller for hair loss. And if you send your starting photo to Dermaroller.pk on WhatsApp, we will keep it and compare it honestly with your six-month one.
